- Your order number.
- Full addresses of the additional SHIP TO locations
- What you would like to have shipped to each location
- Do you want a packing slip enclosed in the package going to the shipping location?
All I ask is that you pay my additional shipping fees. Enter the number of ADDITIONAL shipping locations in the box below. So if you want your original order shipped to two different locations you would enter a quantity of 1 in the box below because you already paid for one standard shipping in your original order.
